The Fort Lauderdale Police Department regrets that you have been a victim of a theft (or lost property). In order to provide you with the best possible service we will take your report by mail.
Please follow the instructions as detailed here:
- Download the form [PDF]
- Print or type the report
- Do not write in the shaded spaces
- You may attach additional property lists and comments as needed
- List all stolen (lost) property individually on the back side (page two) of the report
- Obtain and list all available serial numbers

An offense report number will be assigned when we receive your completed report form. Any appropriate serial number or make or model numbers of stolen (or lost) property will be entered into the national and state database for recovery purposes.
Please be assured if further information regarding your theft becomes available, we will contact you and take the appropriate action to retrieve your property and bring the offender to justice.
